You can often save time and remove the need for completing forms and other additional paperwork by visiting the Account Upgrades page online.


Once you have logged in, click the My Accounts menu and then choose Account Upgrades to review and choose your available upgrades.

Single to Joint Application Form

If you already have a TD Waterhouse Account this form will allow you to add another person to trade on your account and allow you to trade and hold shares in joint names.

Upgrade Form

If you already have an account with TD Waterhouse, you can gain access to a whole range of convenient services and to new markets by upgrading your account online. To do this login now and go to Account Administration/Account Upgrades. Alternatively, you can download and complete the Upgrade Form. Please make sure you have read the Customer Terms of Service.

ISA Transfer Form

Transfer your existing ISA to TD Waterhouse.

Securitised Derivatives Appropriateness Assessment

You can apply for access to trade Securitised Derivatives using our Appropriateness Assessment form, read more about this. All other customers should use the Upgrade Form (PDF 40KB). More information.

Asset Transfer Form

Transfer your assets to TD Waterhouse and benefit from an improved and more streamlined approach to managing your accounts. More information.

International Dealing (Trading, Trading ISA and SIPP accounts)

If you have a Trading, Trading ISA or SIPP with TD Waterhouse you can apply for access to trade international equities by upgrading your account online. Login now and go to Account Administration/Account Upgrades. Alternatively you can download and complete our Upgrade Form (PDF 40KB).

PLEASE NOTE: If you invest in US (United States) stocks and shares, you will need to complete a form W-8BEN. Obtaining this documentation ensures that we can give you any appropriate tax relief on income paid to you from US stocks and shares. We do not send this form to the US Internal Revenue Service, instead we will hold it on our files to confirm that we have given you the correct benefits of any US tax treaties. Crest Transfer Form

If you hold share certificates you can put them into your Nominee Account allowing you to view all your holdings online. You just need to complete and sign a Crest Transfer Form for each stock held and return to us with your share certificates.

The easiest way to do this is to login and use our Crest Transfer Form Generator to create pre-filled Crest Transfer Forms in PDF format which you can then download, print out and sign. Once logged in go to 'Payments and Transfers/Transfer Shares In'. Login now.

Alternatively, download blank CREST Transfer Forms and complete them yourself, returning them to us with the appropriate certificates. Please ensure the details on the transfer forms match exactly with those on your certificate irrespective of spelling mistakes.

Third Party Authorisation Form

This enables you to authorise one or more individuals to deal on your behalf by submitting a completed Third Party Dealing Authority form, signed by all the account holders and the nominated individual. More information.

Book Cost Form

You can add or amend your own book costs online. Once logged in go to 'My Portfolio' and follow the instructions on the page.

Alternatively you can download the Book Cost Form, complete and return it to us and we will update your book costs for you.

Direct Debit Form

If you trade in paper share certificates, you can choose to settle your trades using a Direct Debit from your bank account.
